Massachusetts DIA now requiring Affidavits accompanying Section 19 Agreements for Pro-Se Employees to be notarized

We previously advised you of the Massachusetts DIA’s new policy limiting lump sum settlement agreements to only
one
date of injury, wherein the parties now need to draft
separate lump sum settlement documents

The DIA is now requiring that the affidavit accompanying a Section 19 Agreement with a Pro-Se Employee must be notarized!
